If you would like to request a medical report, please use this form.

All requests for forms/medicals are non-NHS work and therefore chargeable.

You will be informed of the fee prior to the work being completed. 

 

Please note a medical report is not needed for PIP claims/housing/private taxi medicals, instead we can print you a patient summary. Please contact the surgery to obtain a patient summary from our reception team.
